Histological pattern of steroid resistant nephrotic syndrome in children: a single centre study.

Objective: To determine the histological type of steroid resistant nephrotic syndrome in children and to find any deviation from local and international patterns. Material and Methods: This is a descriptive study carried out at the Department of Nephrology Khyber Teaching Hospital, Peshawar from 1st January 2008 to 31st October 2010. Forty five children having SRNS underwent renal biopsy to find the histological type. Results: Out of forty five children, 27 (60%) were male and 18 (40%) were female. Mean age of presentation was 11.26 years ±3.21 SD. Fourteen children (31.1%) had microscopic hematuria and ten (22.2%) children had hypertension. Histopathology revealed focal segmental glomerulosclerosis (FSGS) in 28.8%, IgM nephropathy in 17.7%, minimal change disease (MCD) and membranous nephropathy (MN) in 13.3% each, membrano proliferative glomerulonephritis (MPGN) in 11.1%, mesangial proliferative glomerulonephritis (MesPGN) in 8.8% and IgA nephropathy in 6.6% patients. Conclusion: FSGS was the commonest cause of SRNS. IgM nephropathy is present in one fifth of children having SRNS. Odd features and increasing age are associated with SRNS.
