Various presentations of Omphalomesenteric Duct remnants in children.

Objective: To note various presentations of omphalomesenteric duct (OMD) remnants and their management in children. Design: A prospective study. Place and Duration of Study: Department of Paediatric Surgery, National Institute of Child Health, Karachi from January 2000 to December 2001. Subjects and Methods: Forty cases were managed. They were placed in three groups, those operated electively were put in Group I, patients requiring emergency surgery were placed in Group II and cases found incidentally at other operations made up the Group Ill. Details including epidemiological data and management were recorded on a proforma. Results: In Group I there were nine patients, in Group II twenty one and in Group III ten cases. In group I, seven had patent OMD, one with umbilical sinus and one umbilical polyp. In group II, four cases presented with peritonitis secondary to Meckel`s diverticulitis, six with secondary intussusception and eleven had intestinal obstruction due to bands, four of them had internal umbilical cyst along with band. In group III, ten patients had Meckel s diverticulum as an incidental finding, two of whom underwent diverticulectomy. One patient died in this series. Conclusion: As a group, remnants of OMD are common congenital condition. In majority of cases with intestinal obstruction a provisional pre-operative differential diagnosis can be made. Diverticulectomy at incidental finding should be tailored according to presentation.
