Intracranial Fungal infection in an HIV +ve individual.

A 32 years old man presented with - Weakness of left side of body -1 month, - Fever- 15 days, - Coma - 2 days. In MRI, T2-weighting axial images revealed multiple high intensity space occupying lesion in the brain occupying cerebellar, and both cerebral hemisphere. The lesion is seen in high parietal, high vertex which shows significant edema around it (its measuring nearly 1.5 cm). Patient diagnosed as a case of tuberculous lesion (C.S.F examination was ± treatment started under basis of imaging finding but no response noted. A repeat MRI was done after 3 weeks which again revealed multiple lesion and no significant improvement. C.S.F examination was also normal. Patient was deteriorating clinically. On further inquiring the history patient`s brother gave history of HIV +ve. Then specific treatment were done, patient responded and improved clinically. MRI was repeated after 1 month shows marked improvement and almost total healing of right parietal lesion. The lesion in the brain was diagnosed `Toxoplasmosis` in view of history of HIV.
