Identification of Prescription Writing Errors in Welfare Hospital and Various Private Clinics of Hyderabad City.

OBJECTIVE: To identify and compare the frequency of prescription errors in hand written prescriptions of welfare hospital and various private clinics of Hyderabad city. STUDY DESIGN: A descriptive and cross sectional study. PLACE AND DURATION: Prescriptions were collected from the OPD of welfare hospital and various private clinics of Latifabad, th th Hyderabad. The duration of study was 01 year from 15 Jan 2016 to 26 Dec 2016. METHODOLOGY: A total of 512 prescriptions from Welfare hospital and 561 prescriptions from different private clinics were collected by random sampling method and analyzed as per WHO guidelines of good prescribing manual. All the data were tabulated to identify the frequency of errors and calculated in percentage. RESULTS: The high error rate in prescriber's name (74.4%), patients weight (98.6%), route (79.84%), duration (82.15%), brand names of drugs used (51.2%) in the prescriptions of Welfare hospital whereas prescriber's contact number (98.78%), patient's registration number (100%), strength (68.09%) with dose unit (59.56%), frequency of medication (29.84%) in the prescriptions of various private clinics of Hyderabad. CONCLUSION: It has been observed that major flaws found in prescription writing by the physician in both the settings of health care system.
