Phyllodes Tumor of Breast.

Phyllodes tumors account for 0.3% -0.5% of all the breast tumors. They are usually benign but malignant variants exist and have the potential to metastasize. Surgery is the mainstay of treatment as there is no proven benefit of adjuvant chemotherapy or radiotherapy. Wider excision with 1 cm clear margins is recommended in cases where the resection margins are involved. Simple mastectomy is recommended even for large benign tumors as there is increased likelihood of recurrence. We are presenting a case of 42 years old female with benign phyllodes tumor and a brief review of literature on the subject.

Case Reports: A 42 years of age female presented to our breast clinic with history of lump in right breast for one month. There was no history of mastalgia or nipple discharge or breast disease in the family. She had taken an oral contraceptive pill for 12 years but had ceased this medicine for the past 10 years. Clinical examination revealed a 5 cm mobile mass in the retroareolar area of the right breast. It was firm in consistency, with well defined edges. She underwent mammography and breast ultrasound. Mammography revealed a 5 cm dense, well defined mass deep to the nipple. No areas of microcalcification were seen suggesting the benign nature of the mass. Ultrasound findings corroborated with that of mammography and suggested the lesion to be a fibroadenoma. She underwent an excision biopsy of the mass following a pre-operative frozen section which showed it to be a benign phyllodes tumor (less than four mitoses per ten high power field). The formal histology was consistent with the findings of frozen section. Since the resection margins were involved on one side, wide local excision was carried out and clear margins were achieved. She is undergoing an annual follow-up in the breast clinic.
