Coin in Nasopharynx in a Young Child.

A 5 years old child presented with typical picture of adenoid hypertrophy but surprisingly plain radiograph of lateral view of neck showed a radio opaque object in the line of nasopharynx. During examination under anesthesia, a coin was seen impacted in nasopharynx partially covered with granulation tissue which was retrieved.

Case Report: A 5 years old boy was brought to otorhinolaryngology department of Combined Military Hospital, Okara cantonment with 6 weeks history of nasal obstruction, nasal discharge, occasional hemoptysis and mouth breathing. Defective hearing was noted in school. The child had already received antibiotics, antihistamines and nasal drops with worsening of symptoms. Detailed clinical examination revealed partial nasal obstruction, purulent nasal discharge, obvious mouth breathing and retraction of both eardrums. A provisional diagnosis of adenoid hypertrophy leading to otitis media with effusion was made and plain radiograph of lateral view of neck was requested. Surprisingly radiograph showed a crescent shaped radio opaque shadow in the nasopharynx which was round on anteroposterior view (Figure 1). As the child was highly uncooperative towards posterior rhinoscopy, examination under anesthesia was planned. During examination a coin was found in the nasopharynx, partially covered with granulation tissue that would bleed on touch. The coin was carefully disimpacted and removed. Bleeding was controlled by pressure packing and antibiotic cover was given for five days. The child remained symptom-free during six weeks of follow-up.
