Neonatology is a rapidly advancing field of medicine in Pakistan with many neonatal intensive care units opening throughout the country. The challenging nature of this field of medicine in a resource limited country raises numerous ethical issues such as the lower limit of viability, initiating or not initiating resuscitation at birth of preterm or a baby suffering from significant birth asphyxia. This paper discusses the dilemmas frequently faced by clinicians and parents alike. It presents a best practice template on how to make ethical decisions interdependently between the clinician and the parent or family of the baby.
