Impact of single layer or double layer closure on uterine rupture in subsequent labour.

Objectives: The purpose of this study was to assess the impact of single layer or double layer closure on uterine rupture at subsequent delivery. Methodology: The study was conducted at the department of Obstetrics and Gynaecology, Nishtar Hospital, Multan from January 1996 to December 2001. In this five years study 21429 deliveries were recorded. 1041 patients with trial of labor were evaluated. Single Layer Closure (SLC) technique was used in 457 (43.9 %) and Double Layer Closure (DLC) technique was used in 584 (56.1%). Results: Vaginal delivery was achieved in 802 (77.04%), caesarean delivery in 225 (21.6%) and uterine rupture observed in 14 (1.34%) of 1041 women. A significant difference on uterine rupture seen between the two groups p<0.004. Single layer with vicryl No.1 gave as good strength as double layer chromic catgut closure less operative time and morbidity observed with single layer uterine closure. Conclusion: Single layer uterine closure with polycolic acid suture gives as strength to uterine scar as double uterine closure. In addition there is short operative time and less postoperative morbidity observed in single layer closure.
