Comparison of Primary and Secondary Antimicrobial Susceptibility Testing of Aerobes isolated from Blood Cultures.

To determine the reliability, accuracy and feasibility of early primary sensitivity of positive blood culture, we compared the results of primary (direct) sensitivity testing with secondary sensitivity testing. For 89 Gram positive aerobic isolates, over all agreement between two methods was 97%,out of total 1335 comparisons. There were 9 very major, 2 major and 28 minor discrepancies. Majority of discrepancies were noted with staph. aureus and coagulase negative staphylococci. For 111 Gram negative bacilli, overall agreement between primary and secondary sensitivity testing results was 92.2% out of 1554 comparisons. There were 10 very major, 1 major and 45 minor discrepancies. Pseudomonas aeruginosa, Enterobacter cloacae were accounted for most of the dlcrepanctes. The results Indicate that early sensitivity reporting directly from unimicrobial positive blood culture broth is generally both feasible and accurate.
