Frontal temporal dementia (FTD) in a Charsada family.

A forty seven years old right handed woman, school teacher from Charsada, was referred because of two year history of gradually increasing difficulty in finding names. She was able to describe events in general terms but was unable to specifically name individuals or objects. She knew very little of her job and had been experiencing speech difficulty for about one year. She had a compulsive repetition of a word or a syllable echolalia, outburst of rage and disinhibition. She also had a personality change in that she would seem to persist on asking her brother about his sexual life from which it was difficult to distract her. Family history was remarkable for her father and paternal grandfather having probable Alzhiemer`s disease that was late onset. On examination the patient scored 19/30 on the Mini-Mental State Exam (MMSE). On testing higher cotical functions her speech was some what rambling and hesitant with word finding difficulty. She had compulsive repetition of word or syllable. She scored 102/144 on the Dementia Rating Scale which reflects a moderate severity of dementia. She scored in the normal range on digit span forward and backward (tests of immediate attention), in copying complex figures (tests of visuospatial skills. She scored in the impaired range on tests of verbal and visual memory, naming and verbal fluency. Tests that require divided attention and problem solving skills were also impaired. There were no cardiovascular abnormalities. Blood pressure was 140/80. Cranial nerve examination and general medical examination was normal. Computed tomography (CT) of the brain showed enlarged lateral ventricles and cerebral atrophy. On magnetic resonance imaging (MRI) scan showed bilateral anterior temporal lobe and frontal atrophy.
