aslamoalikum iam final year medical student..i want to discuss a gynecological problem of my friend and wants to seek ur help..
Actually she is suffring from vaginal bleeding since manarche she is now 23 years old she continuely bleed for the whole month ..she had visited several doctors at lahore and other cities done every test like ct mri fsh lh bt pt tsh ects all are normal ...the doctors precribe ocp for her she is taking the medicine from last 8 years in interval when she left medicine the problem again statrted .......the doc advices not to take medicine in a continuos way but in interval.....some doc says its a complecatedet case and resolves after marrige .....some say this and some says this ........at the end the doc says its due to hypo function of the ovaries which get mature after two years after menache but surprizingly its does not mature .......plz help me in tis regard ...i will be very thank full to u .....waiting for ur replyies..

Can you please mention the results of her FSH, LH, Estradiol (at 2nd day of menstural cycle if possible), Progestrone on day 21 (if possible), Ultrasound ovaries, TSH / FT4 and related investigations.
Lets see what we can do for her.

sir i have asked her ab these scans and tests ..she told me that fsh and lh are low becuse of taking ocp and all the rest of the test are normal ....currently do not have reports with her ...

she had done FSH LH PROLECTIN T3 T4 TSH ALL ARE NORMAL EXCEPT LH AND FSH AND DOC SAYS ITS DUE TO OCPS

A good Estradiol level will tell that ovaries are intact and ovarian follicles are maturing. If you study the menstural cycle, estrogen makes the follicular phase of uterus (prepares the uterus for ovum reception) and on day 14 onwards, progesterone released from the corpus luteum (maximum on day 21) causes proliferative phase. If there is no ovum, then on day 27-28, progesterone suddenly decreases, resulting in the necerosis and degeneration of the uterus and menstural bleeding. Menstural cycle is regulated by Gonadotrophins and if they are suppressed by some way, endogenous estrogens are inhibited as well. One way is to stop the menstural cycle is by inhibiting gonadotrophins. Has anybody tried giving her GNRH (goserelin, etc), thus supressing all her ovarian activity and mensturation?
