Frequency of Dystonia Among Post Stroke Patients in Pakistan.

Objective: To find out the frequency of dystonia and its types among post stroke patients at different hospitals of Rawalpindi and Islamabad Study Design: Cross Sectional Survey. Place and Duration: The study was conducted at different hospitals of Rawalpindi and Islamabad from August 2015 to March 2016. Materials and Methods: 387 patients were recruited through non probability purposive sampling. The inclusion criteria was individuals; having age group more than 18 years of age, experiencing first episode of stroke, presenting with both ischemic or hemorrhagic stroke, having stroke for at least three months at time of presentation. A self reported questionnaire was used to collect data after informed consent. Data was analyzed using Microsoft Excel 2007 and SPSS (V.20) Results: The study population comprised of 51.6 % male and 48.4 % female patients. There were 33 % who belonged to age group of 46- 55 years. The percentage of patients who developed post stroke dystonia was 25% (97 patients). Among this population 19% (68 patients) had spastic dystonia and 6% (29 patients) presented with various other types of dystonia. Out of these 16 patients had hemidystonia, 10 had focal dystonia and just 3 had segmental dystonia. The patients having stroke from 1-2 years were found to be having highest frequency, 37, of dystonia in it 27 patients developed spastic dystonia while 10 developed dystonia of other types. Conclusion: The prevalence of movement disorders after stroke is low as reported by previous researches. These disorders can present early after stroke or can be delayed in their presentation. Spastic dystonia is more frequent form occurring in majority of patients. Most of the patients found with dystonia were having post stroke duration between 1 – 2 years which showed delayed development of dystonia after stroke.
