J Pak Assoc Derma Nov ;12(4):190-3.

Amyloidosis is a syndrome of multiple etiologies. Systemic amyloidosis occurs as a result of amyloid deposition in various tissues. The amyloid fibrils in systemic amyloidosis have been reported to originate from immunoglobulin light chains. We studied two cases of systemic amyloidosis. One presented with renal failure, and the other with malabsorption.

Case Report: Patient 1 was a 50 year old male who presented with a history of skin rashes, and fatigue for two years, and hoarseness and loss of weight for last two months. On examination purpuric and papular lesions were seen on eyelids, axillae and neck. There was macroglossia, oedema of feet and marked proteinuria. Serum creatinine was 4.70mg% and blood urea 171 mg%. Urine protein electrophoresis was consistent with a monoclonal gammopathy. Biopsy from skin lesions showed features of amyloidosis. Renal biopsy showed amyloid nephropathy. Bone marrow aspiration showed hypocellularity.
