Acute Hepatitis E: experience at Holy Family Hospital, Rawalpindi.

An alarmingly high acute hepatitis-E virus (HEV) related mortality in non pregnant group is presented in this cross sectional observational study which was conducted at Gastroenterology and Liver Section of Holy Family Teaching Hospital, Rawalpindi, from February to August 2005. 24 patients of acute hepatitis (clinical, investigational features of acute hepatitis plus IgM-HEV positivity) were included. All patients were managed in a standard way. Their age, gender, presenting and clinical features, ALT, serum bilirubin levels, platelet counts and outcome (discharge after recovery or death) was noted on a specifically designed proforma. Of the 24 patients, 66.7% (n=16) were male and 33.3% (n=8) female. 50% females were pregnant. Mean patient age was 27.71±11.8 years. Jaundice (95.8%), vomiting (70.8%) and fever (66.7%) were commonest presenting complaints. Hepatomegaly (33.3%), splenomegaly (12.5%) and ascites (4.1%) were common signs. 37.5% (n=9) patients had acute fulminant hepatic failure. 33.3% (n=8) patients all having acute fulminant hepatic failure expired while rest recovered. 25% (n=1) pregnant patient expired.
