Laparoscopic versus open Appendicectomy as an elective procedure.

OBJECTIVE: To determine the role of laparoscopic versus open appendicectomy in current surgical practice and to assess the postoperative morbidity, duration of hospital stay, technical feasibility and cosmetic results of particular procedure. This study is designed to compare the laparoscopic appendicectomy with open appendicectomy in elective setup only because of unavailability of laparoscope in emergency. MATERIAL AND METHOD: This study has been conducted in Ward-26, Surgical Unit-III, Jinnah Postgraduate Medical Centre, Karachi over a period of two years from February, 2004 to February, 2006. DESIGN: This is a quasi experimental non-probability purposive study. RESULTS: Out of 40 patients 20 under went laparoscopic appendicectomy and 20 open appendicectomy. The mean duration of surgery in laparoscopic appendicectomy was 71.4 + 11.67 minutes as compared to 36.85 + 10.15 minutes in open appendicectomy. The hospital stay in laparoscopic appendicectomy was 1 day as compared open appendicectomy which was more than 1 day. One to two doses of intravenous analgesia were needed in laparoscopic appendicectomy patient as compared to 3 – 4 doses in open appendicectomy, where as wound infection was seen in 10% cases in laparoscopic appendicectomy and 20% in open appendicectomy. There was minimum surgical scar i.e. 5% in laparoscopic appendicectomy as compared to 25% open appendicectomy. CONCLUSION: Laparoscopic appendicectomy offered a relatively more advantage over open appendicectomy, although it took longer time and it is also expensive. The choice of procedure depend on surgeon or patients preference.
