Rickets is disorder with diverse etiologies characterized by lack of normal mineralization of osteoid tissue more prominently at growing ends of long bones. Defects are concerned primarily or secondarily with calcium and phosphorous metabolism. CASE REPORT: Presenting at the age offour and treated till then as nutritional rickets with multiple defonnities nonnal serum calcium and elevated alkaline phosphatase. Urinary phosphate` was raised and an alkaline urine with nonnal plasma anion gap. Arterial blood gases excluding renal tubular acidosis. Serum calcium and phosphorous ratio as well as calcium/ creatinine ratio established. Vitamin 03 metabolite studies illustrating `Hypophosphatemic Rickets`. DISCUSSION: Definitive management ofHypophosphatemic rickets and latest developments on mechanisms involved. Newer treatment options for renal tubular acidosis a clear approach to its management.
