Serum PSA as a predictor of prostate volume and surgery in BPH-related acute urinary retention.

Objective: To establish the correlations of serum Prostate Specific Antigen (PSA) in men presenting with urinary retention due to BPH and prostate volume. Design: Prospective observational study. Material and Methods: Thirty patients having retention of urine due to BPH were studied over a period of one year in the department of urology, Lahore General Hospital, Lahore. Serum prostate specific antigen (PSA) estimation was carried out on blood taken before urethral intervention (digital rectal examination or Foley Catheterization). Prostate volume was measured by ultrasonography. Trial without catheter (TWC) was given in all cases. Prostate surgery was done in those patients where TWOC failed. Results: In thirty patients, age range was 50-79 years serum PSA ranged from 0.4ng/ml to 6.0ng/ml with mean PSA value of 2.81ng/ml. Prostate volume ranged from 32 to 70 ml. Majority of patients had prostate volume in the range of 35-65ml. Twenty seven (89.1) patients needed prostate surgery. Conclusion: Direct correlations were seen between PSA and age of the patients. Higher PSA value also indicated higher prostate volume. Serum PSA value and prostate volume are a predictor of acute urinary retention and need for prostate surgery.
