Effects of Dobutamine infusion following volume loading compared with volume loading alone to improve hemodynamics in right ventricular infarction.

Objective: To determine efficacy of dobutamine infusion Vs volume loading in improving the hemodynamics in Severe RV infraction. Design: Prospective randomized, comparative single centre study. Setting: Emergency department Mayo Hospital Lahore. Subject & methods: Forty randomized patients with severe RV infarction 30 male and 10 female mean age 55±10 years with base line BP (systolic) 70±5 heart rate 58 ±10/min right artial pressure 13±2 mm Hg, were given 400 mls normal saline I/V over 5 minutes and then after returning hemodynamics to base line again given dobutamine infusion at the rate of 10 ug/kg/min and BP(Systolic), heart rate and right artial pressure recorded again. Results: Total 40 patients were included in the study. Effect of volume loading and dobutamine infusion was observed on each patient. The base line heart rate increased from 58±10/min to 60±12/min(p > 0.1). systolic BP 70±5 mm Hg to 75±5 mmHg (p<0.05) right artial pressure 13±2 mmHg to 18±3 mm Hg (p<0.001). Effect of dobutamine infusion increased heart rate 59±11/min to 70±12/min (p <0.005), systolic BP 70±5 mmHg to 80±10mmHg(p<0.005) right atrial pressure 14 ± 3 mm Hg to 15 ±2 mm Hg (p<0.01). Conclusion: Dobutamine infusion following an initial volume loading augments the hemodynamic improvements in severe right ventricular infarction as compared to Volume loading alone.
