Incidence of pharyngocutaneous fistula after total laryngectomy.

Objectives: 1. To find the frequency of pharyngocutaneous fistula after total laryngectomy. 2. Management of pharyngocutaneous fistula after total laryngectomy. Setting: Department of ENT and Head & Neck Surgery, Jinnah Hospital, Lahore. Sample size: This was based on past records. This included all cases which come during our study which was not less than 30 cases. Methods: Data was collected according to proforma. After collecting clinical and diagnostic data were discussed in ward. These cases were presented before teaching staff of ENT. After discussion, collective decision was made to formulate treatment policy. Then patients were treated accordingly and followed to know about the outcome and complications of surgery. At the end of study, data was compiled, and presented in this work. Result: The majority of patients 16(53.3%) have Haemoglobin 12-12.9 mg/d1 The Haemoglobin range 11 to 14.2 and mean±SD 12.78±0.7 in all 30 patients. At 1st week follow-up, the pharyngocutaneous fistula were present in three patients (10.0%). At 2nd week only one fistula was diagnosed (3.3%). Conclusion: Most fistulas can be successfully managed with conservative treatment. Except for rare cases in which large defects are present. Repair is appropriate treatment when conservative treatment has failed.
