Ruptured falciform ligament abscess – a rare cause of intestinal obstruction.

Falciform ligament abscess is a rarity. We report a case of 02 month old girl who presented with signs and symptoms of mechanical mid gut obstruction. Her x-ray abdomen showed multiple air fluid levels. After resuscitation the patient underwent coeliotomy, which revealed an acute kink in the proximal ileum and collapsed distal intestine. There were thick flakes of pus on the serosal surface of the distal jejunum and proximal ileum. The area of the kink was adherent to the falciform ligament which was swollen and inflamed. On separation of gut from the ligament beads of pus started coming out from an irregular perforation at the lower border of falciform ligament. A diagnosis of ruptured falciform ligament abscess resulting in hair pin loop adherence of ileum to site of perforation was made. Falciform ligament was excised in toto and alignment of the gut restored. The patient made an uneventful recovery & remains healthy upto a 06 month post-op follow up. To our knowledge rupture of falciform ligament abscess causing intestinal obstruction has not been reported in world literature.
