Complications of central venous catheter insertion in cardiac surgery patients: comparison between internal jugular and subclavian approach.

OBJECTIVES: To determine the incidence of complications after Central Venous Catheterization (CVC) in the patients undergoing cardiac surgery and to identify the factors associated with these complications, and to compare between Internal Jugular (IJ) and Subclavian (SC) approaches. STUDY DESIGN: Study was a prospective, interventional, randomized study. PLACE & DURATION: Study was conducted in the department of Anesthesia of Civil Hospital (CHK) and Dow university of health sciences Karachi (DUHS),over a period of one year. PATIENTS AND METHODS: Prospective randomised study of cardiac surgical patients was carried out two groups were formed. Group A (n=62) received JJ catheterisation and Group B (n=60) received SC cathetarisation Patients were observed for mechanical and catheter related complications in the operating room and followed on into the intensive care unit. Body temperature was regulated. Chest X-ray was routinely performed on all patients and in section site was daily examined. RESULTS: SC group had two major mechanical complications while non was recorded in IJ group. Ratio of minor mechanical complications between the two groups was the same. Complications were more among the female population as against the males. CONCLUSION: This study concludes that when performed by the reasonably experienced person, IJ approach is associated with lesser or no major complications, although the incidences of minor complications is slightly higher and are easily correctable and benign. Incidences of complications is higher in female and in pts over the age of 50.
