Incidental prostatic cancer after suprapubic adenectomy.

OBJECTIVES: To evaluate the prevalence of incidental carcinoma in BPH after suprapubic adenectomy. STUDY DESIGN: Prospective observational study. PLACE AND DURATION: Department of surgery and Pathology, Nawabshah Medical College& Hospital, Nawabshah, from June 2002 to June 2005. PATIENTS: A total of 120 patients who were presumed to have BPH and underwent suprapubic adenectomy. METHODOLOGY: Detailed data of the patients was collected. All the patients with lower urinary tract symptoms secondary to presumed BPH, underwent suprapubic adenectomy and histopathological analysis for incidental prostatic cancer, have been included in the study. OUT COME MEASURES: Age, clinical presentation, histological appearance and grading. RESULTS: 13 patients (10.83%) had incidental prostatic adenocarcinoma. The clinical presentation in benign disease was retention of urine in 54 patients (45%), prostatism in 51 patients (42.5 %) and haematuria with prostatism in 2 patients ( 1.66%), while in incidental carcinoma group 10 patients (8.33%) had retention and 3 patients (2.5%) had prostatism. Five patients (4.16%) had well differentiated, 6 patients (5%) had moderately differentiated and 2 patients (1.66%) had poorly differentiated tumors. CONCLUSION: There is no irrefutable method of clinically diagnosing early carcinoma of the prostate. Open prostatic biopsy is the most definite modality evaluated.
