Management of Complete Posterior Urethral Stricture by Optical Urethrotomy - A Preliminary Study.

Optical internal urethrotomy was performed in fifteen patients with traumatic complete posterior urethral stricture. Mean age was 38.3 years. Commonest cause observed in all patients was fracture pelvis with or without associated injuries. The importance of antegrade cysto-urethrogram and retrograde urethrogram in the assessment and selection of patients for minimally invasive procedure is highlighted. Length of stricture could not be ascertained in 33.3% cases by contrast study. Stricture was graded as soft in 46.6%, hard in 33.3% and very hard in 20 % patients. Eighty percent patients had stricture less than 2 cm in length. At one month follow up of all 15 patients, the clinical response was good in 10(66.6%) patients, satisfactory in 4(26.6%) patients and poor in 1(6.6%) patient, whereas, at nine months follow up of thirteen available patients, the response was good in 9(69.2%), satisfactory in 3(23%) and poor in 1(7.6%) patient. Endoscopic management of traumatic urethral stricture appears to be a minimal invasive approach to a difficult problem with satisfactory results. However, a longer follow-up and strict patient compliance is mandatory to establish the confirmation of this modality as the best procedure
