Laser surgey in bilateral abductor vocal cord paralysis.

Objective: To prove safety, efficacy, cost effectiveness, reduced hospital stay and minimum complications rate with use of CO2 laser in managing the patients having bilateral abductor vocal cord paralysis with airway obstruction and voice problem. Design: A prospective study. Place & duration of study: The study was conducted at the Department of ENT unit 1, Allama Iqbal medical College / Jinnah Hospital, Lahore from May, 2008 to April, 2009. Patients & Methods: A total of fifteen consecutive patients with bilateral abductor vocal cord paralysis causing airway obstruction were included in the study. All were having tracheostomy already. All patients underwent unilateral partial arytenoidectomy and posterior cordectomy. Results: All patients were successfully decannulated with adequate airway for normal routines of life and having fairly good quality of voice and minimal complications rate. Conclusion: Laser partial arytenoidectomy with posterior cordectomy is recommended in patients with bilateral abductor vocal cord paralysis as it is a simple, minimally traumatic, reliable procedure with minimal complications.
