What to do with Gartland type III supracondylar fractures of humerus in children- study of 84 patients.

Objective: To evaluate the clinical results after fixation of Gartland type III supracondylar fracture with two cross K wires operating within 2 weeks after initial injury. Study Design: Prospective Place and Duration of Study: Rawalpindi General Hospital and District Head Quarter Teaching Hospital, Rawalpindi, from 2003-2007 Patients and Methods: Eighty-four patients of Gartland type III supracondylar fractures of humerus were included in this study. All these injuries were operated through lateral approach within 2 weeks of initial trauma, patients between 2-10 years of age were managed and included in the study. Associated injuries were also treated during the study. All those patients with type III supracondylar fractures of humerus were operated through lateral approach and fixation done with 2 cross K wires. Results: It was seen during the study that 77.4 % of patients had excellent results, Good results were seen in 14.2 % of patients and 8.4 % of patients had fair results. Conclusion: Early rehabilitation after surgery by active elbow exercises has produced best results in our study. Cross K wires fixation creates 3 points fixation of the fractures that allows early motion of the joint and does not require the application of plaster backslab after operation. Achievements of full range of motion was the main goal of study and it was achieved within 8 weeks after surgery.
