Comparison of dinoprostone (PGE2) and misoprostol (PGE1) for induction of labor.

OBJECTIVE; To compare the safety, efficacy and cost effectiveness of vaginal Misoprostol (PGE1) with vaginal Dinoprostone (PGE2) for induction of labour. METHODS; It was a comparative study for a period of 6 months. This study was carried out in the department of Obstetrics & Gynecology, Jinnah Hospital, Lahore. Patients admitted in labour ward for induction of labour were randomly enrolled in two groups. Group A patients included 100 patients and were induced with Tab Dinoprostone (PGE2) vaginally. Group- B patients were induced with Misprostol (PGE1) given vaginally. The main parameters measured were duration of labour, need for augmentation, mode of delivery, prevalence of fetal distress; failed induction/ failed progress were analyzed among both groups. RESULTS; Misprostol (PGE1) was found more effective than Dinoprostone (PGE2) in producing cervical changes and induction of labour. In Group A (Dinoprostone), 59% patients delivered vaginally, while 70% in Group B (Misoprostol), showing high efficacy and successful induction. Hyper-stimulation occurred only in one multigravida with PGE1 with resultant fetal distress, for which cesarean section was performed. The cost of induction was remarkably less in group-B (PGE1) patients as compared to Group-A (PGE2). CONCLUSION; In this study vaginal misprostol has been found more effective, reasonably safe and less expensive for induction of labour.
