Spontaneous Bacterial Peritonitis in Cirrhosis with Ascites - An Experience at PMCH Nawabshah.

Patients of Cirrhosis with Ascitis were Prospectively evaluated for the presence of Spontaneous Bacterial Peritonitis (SBP). 50 patients after exclusion criteria were grouped into SBP (60%) and NON SBP (40%). Groups on the basis of Ascitic Fluid Examination. - 56.25% of the Pt, were in classical SBP, while 18.75% found into CNNA & Bacterasites group respectively. - As per Child`s Criteria 28.12% in class A while in B & C 34.3% & 37.5% were seen respectively. - High mortality observed (70%) in child class C. - Jaundice (81 %), abdominal pain (78.12%), fever, (46.8%), hepatic encephalopathy (71.8%), Tenderness (87.5%), haematemesis (59.3%), malena (65.6%) were common clinical features in SBP Group. - Mean ascitic fluid protein level found low in SBP group as compared with NON SBP group (1.4gm v/s 2.2gm%). - Bed side inoculation method for Ascitic Fluid culture was used to get high yield. - E. Coli (65.3%) found the common Pathogen while salmonella (7.69) in Gram -ve group proved to be uncommon organism involved.
