Outcome of graham's repair in duodenal ulcer perforation: an experience of 96 cases in West Surgical Unit of Mayo Hospital Lahore.

Background: Duodenal ulcer perforation is one of the frequent surgical emergencies faced by the general surgeons. Patients usually present with signs of peritonitis. The most frequent surgical procedure performed for perforated duodenal ulcer is Graham’s omental patch after laparotomy. Our purpose was to see the effectiveness of Graham’s repair. Objective: To see the outcome of Graham’s repair in Duodenal Ulcer Perforation (DUP). Settings: This study was conducted at the department of West surgical, Mayo Hospital Lahore. Study design: It is a longitudinal prospective study. Methodology: 96 cases were included on consecutive bases in 2 years from March 2008 to February 2010. According to the clinical conditions of the patients they were resuscitated and operated in the emergency. Patients with DUP were selected after confirming the diagnosis per operatively. The patients were followed up for 90 days and the outcome was observed in terms of recovery and complications. Data was analyzed by using SPSS system. Results: Among the 96 patients there were 80 (83.33%) males and 16 (16.77%) females. The average age of patients was 38.23±4.67. 94 (98%) patients were successfully operated and 2 (2%) patients died during surgery. During follow up complications developed in 18 (18.75%) patients only in which 6 (6.3%) patients had wound infection, 4 (4.2%) had pulmonary complications, 2 patients (2.1%) had dehiscence, leakage / fistula was seen in 1 patient(1%) and secondary hemorrhage in 1(1%) patient. Other complications UTI, DVT and IHD were seen in four patients (4.2%). The cause of their death was late presentation and multiple organ failure after the procedure. The success rate was statistically significant, p-value < 0.000. Conclusions: The success of Graham’s repair was seen in 95.83 % patients.
