Some young patients with essential hypertension have palpitations, sweating, tachycardia and an increased cardiac output suggesting increased secretions of catecholamines. Normotensive persons may experience anxiety attacks that include all the symptoms of excessive adrenergic secretions. However paroxysmal nature of these features with excessive rise in blood pressure should alert the clinician to search for a secondary cause of hypertension. We report a case with such features in a twenty year old male.
