Management of dengue hemorrhagic fever.

Dengue hemorrhagic fever (DHF) is a serious form of dengue virus infection. Third space fluid leak is characteristic of DHF. Prognosis depends upon early detection, meticulous monitoring and judicious fluid management. Material and method: This case series was conducted at department of Pediatrics, Services Hospital Lahore, Pakistan during 2011. Patients from birth to 12 years of age with positive Dengue IgM and evidence of DHF were included. The patients were managed with strict monitoring and fluid management during the critical phase. Results: Out of total 20 cases 45% required intravenous infusion for more than 24 hrs. While 35% were given infusion for 24 hours and 20% were shifted to oral fluid within 24 hours of admission. Out of 20 patients 50% required no bolus, 30% were given single bolus where as 20% required more than 1 bolus. All patients were given boluses of crystalloid solution and only 15% children required bolus of colloid solution and in 25% of cases blood or blood products were transfused. All patients were discharged without any complications. Conclusion: Complications and mortality can be reduced among patients with dengue shock syndrome/ dengue hemorrhagic fever with vigilant monitoring and judicious fluid management; without transfusing large amount of blood and blood products
