An Experience of Cirrhotic Hepatic Encephalopathy at DHQ Teaching Hospital.

Background: A prospective, cross sectional study to note general features, precipitating factors and outcome of cirrhotic hepatic encephalopathy. Methods: Seventy patients with established cirrhosis who presented with hepatic encephalopathy were evaluated and treated in the Department of Medicine DHQ Teaching Hospital, Rawalpindi from July to December 2001. Results: Thirty seven (52.8%) patients were male, and 33 (47.2%) female with mean age 47.67 years. Child-Pugh scoring was done in 50 (71.4%) patients, 66% (n=33) of these were in Child class C. Constipation and upper GI bleed were commonest precipitating factors (32.9 and 31.4%). 87.1% patients recovered, 10%, died, and 2.9% left against medical advice. Mean time to come out of encephalopathy was 41.38 hours. HBsAg and anti HCV antibodies were positive in 4 (14.28%) and 34 (97.14%) of 28 and 35 patients in which these were sought. 20% patients were diabetic. Conclusion: Cirrhotic hepatic encephalopathy is commonly precipitated by constipation and upper GI bleed. Hepatitis C virus is commonest cause of cirrhosis in these patients.
