Diagnostic Accuracy of Fine Needle Aspiration in Confirmed Cases of Tuberculous Lymphadenopathy in Children.

Background: Lymphadenopathy is the one of the common clinical presentation in children. Objective of the study was to determine the diagnostic accuracy of fine needle aspiration (FNA) in confirmed cases of tuberculous lymphadenopathy in children. Methodology: This descriptive prospective study was conducted in Professor S. M. K. Wasti Pediatric unit of Allama Iqbal Memorial Teaching Hospital/Khawaja Muhammad Safdar Medical College, Sialkot from January 2012 to September 2013. We enrolled all the children upto 15 years of age with lymph node enlargement (size of lymph node more than 2 centimeter) and duration of the disease more than two weeks. Every child was subjected to FNA. Every third case was randomly sent for surgical excision biopsy for histopathology. Results: A total 99 cases were subjected to FNA of lymph nodes during study period. Since in two patients the aspirate was inadequate, so we discarded these cases and remaining 97 were analysed. Randomly, 33 cases were biopsied with surgical excision. Cervical lymph node accounted for 90 cases (93%). TB was the most common pathological lesion (80%) cases followed by reactive hyperplasia (18%). Sensitivity, specificity, PPV, NPV was 100%, 77%, 92% and 100% respectively. Conclusion: FNA biopsy for cytology is a simple, inexpensive, accurate, less invasive and first line screening method to diagnose the pathological lesions in paediatric lymph nodes.
