Conversion of malpositioned ankylosed hip of severe flexion & abduction position of longstanding duration with Total Hip Arthroplasty (THA), needs a proper preoperative planning and arrangement of armamentarium and implant to avert any untoward incident.The muscular atrophy especially Glutei, Extensors and Flexors of hip is a real challenge to cover in postoperative period. The abductor and extensor weakness in these patients may often lead to dislocation and life long dependence on walking aid. We are presenting here one such case, who presented with a grossly malpositioned ankylosed hip, regained all his movements in hip, vigour and muscular strength, thirty-five years after his hip was fused to get rid of his severe painful hip due to secondary osteoarthritis.
