Boytchev II procedure for the treatment of recurrent anterior shoulder dislocation.

Objective: To evaluate the effectiveness of Boytchev II procedure in the management of recurrent anterior shoulder dislocation. Study Design: Descriptive study. Place And Duration Of Study: Department of Orthopaedics, Lady Reading Hospital, and Said Anwar Medical centre, Dabgari Gardens, Peshawar,September, 2004 to August, 2009. Subject and Methods: From September 2004 to 2009, Boytchev II procedure was performed in 14 cases of recurrent anterior dislocation of the shoulder joint. Data was evaluated for side of dislocation, number of dislocations, post operative wound infection, and pre and post operative range of motion, recurrence, and arthritis of glenohumeral joint. All patients were sufferingfrom recurrent anterior shoulder dislocation for the period of 2-6 years after traumatic dislocation of shoulder joint. The minimum follow up was 2 years but majority of patients were followed for a period of up to 6 years. The patients were scrutinized by range of motion in terms of external rotation at 900, abduction, forward flexion and shoulder stability by shoulder apprehension test, compared with pre operative evaluation. Post operative shoulder stability and range of motion were compared to preoperative status. Results: The numbers of dislocations after initial injury in these patients were between 6-20 with mean (15.79 ± 3.96). The affected shoulder was right in eight cases and left in six cases. The recurrent dislocations were mostly found in young adults male .Ten shoulders were dislocated at presentation whereas four were diagnosed on history and clinical evaluation. On the last post operative follow up visit, one patient was found with limited range of motion, three shoulders improved mildly whereas remaining 10 patients achieved full range of motion. Among 14 patients, three developed superficial wound infection which was treated with dressing and oral antibiotics whereas the remaining 11 had no infection .Complications like recurrence, glenohumeral arthritis and loosening or migration of coracoid screw was not recorded in any case. Conclusion: Boytchev II procedure is an effective and simple procedure for the management of recurrent anterior dislocation of shoulder.
