Treatment of distal tibia fibula fractures with intramedullary nailing & plate fixation.

Objective: To assess the results of intramedullary interlocking nail and plate fixation in distal metaphyseal fractures of tibia fibula Place and Duration of Study: The study was conducted from October 2007 to July 2009 at Orthopaedic Unit I Dow University of Health Sciences & Civil Hospital, Karachi Study Design: Descriptive Study Patients and Methods: 12 male and 06 females (mean age 35 years) with concurrent fractures of the distal Tibia fibula under went intramedullary interlocking nailing in tibia and plate fixation in fibula. 12 close fractures while 06 open fractures with Gastillo type I & II. Results: The mean follow up duration was two years the mean time for bone union 24 weeks. In 04 patient bone grafting done and 03 patients under went dynamization. 01 patient had malalignment with no rotational deformity. No patient had shortening and deep seated infection. 02 patients got screw breakage while two patients had superficial infection at the site of distal locking bolts. Joint stiffness was found in 03 patients, 02 ankle and 01 knee joint involved. Conclusion: Intramedullary interlocking nailing and plate fixation for fractures of distal tibial fibula is effective in preventing malalignment. Plate fixation for fibula provides additional stability even when a single distal locking bolt is used to fix the intramedullary nail to the tibia.
