Treatment of adolescent Coarctation with stent implantation.

We present our experience with a new Cheatham-Platinum (CP) stent and double ballon BIB catheter in two adolescents who had congenital coarctation of aorta. Two female adolescents both age 18 years with congenital CoAo are included in our report. First patient had diagnosis of hypertension made at the age of 15 years, she had a history of Joint Pains and elevated ESR. Further investigations at age 18 years revealed that she had Systemic Lupus erythematosis and additionally congenital coarctation of aorta. Because of the Systemic Lupus Erythematosis she was treated with immunotherapy which involved prednisolone, methotrexate and cyclophosphomide. She was also administered antihypertensive medications for hypertension. Clinically her weight was 54 Kg and right arm blood pressure was 145/95 mmHg. The Doppler Echocardiography showed a pressure gradient of 50 mmHg at the coarctation site which was situated beyond the left subclavian artery. At Cardiac Cartherization the ascending aortic pressure was 160/95 mmHg while gradient at the Co site was 70 mmHg. Angiography of the aorta showed a discrete Coarctation of the aorta distal to the left subclavian artery. Quantitation of the aortogram showed the transverse arch diameter was 15mm, descending Ao at diaphragm measured 19mm in diameter and the Co site was 4 mm.
