Clinical Scoring in the Diagnosis of Acute Appendicitis.

A prospective study on patients of Acute Appendicitis was carried out in emergency department of Allied Hospital, Faisalabad from October, 1998 to December, 1999. Patients with pain in the right lower abdomen were examined on two occasions pre-operatively. The initial examination was conducted by different teams comprising junior surgeons and a senior surgeon, and the decision was made in 110 cases. All these cases were subsequently analyzed according to the modified Alvarado Score, and each case was assigned a particular score. Patients with a score of 7 or above were categorized as group A and patients with a score of six or below as group B. All these patients had a histophatological examination of the appendices removed. There were 97 patients in Group A and 13 in Group B. Out of the former, 95 had histological confirmation of acute appendicitis ; whereas in the latter group 04 had a positive histological report and in 09 cases there was no evidence of acute inflammation of the vermiform appendix. It was concluded that the modified Alvarado scoring is superior to clinical examination alone and is associated with a lesser incidence of histologically negative appendicectomies.
