Outcome of the modified Putt i-Platt procedure for recurrent anterior shoulder dislocations.

Objective: Objective of this study was to evaluate the outcome of modifi ed Putt i-Platt procedure for recurrent traumatic anterior shoulder dislocation. Materials and methods: Th is retrospective study was carried out at the department of Orthopedic and Spine, Hayatabad Medical Complex Peshawar from September 2008 to September 2014. Th irty six patients were included in this study. All these patients were treated with modifi ed Putt i-Platt procedure and were followed up at 06 weeks, 03 months, 06 months and then yearly thereaft er. Shoulder instability was assessed by using Rowe scoring system. Loss of range of motion postoperatively was compared with normal shoulder. Results: Out of 36 patients thirty fi ve (94.4%) were male. Th irty (83.3 %) patients had dominant side aff ected and thirty two patients (88.9%) were right handed. Twenty two (61.1%) patients had their initial dislocation due to sports injury. Th e mean Rowe score signifi cantly improved from 23.42 preoperatively to 81.69 postoperatively, with 31 patients having score greater than 74(86.11%).Loss of external rotation was more than rest of range of motion compared with the normal shoulder. Th e aff ected shoulder lost an average of 11 degree of abduction, 09 degree of elevation, 15 degree external rotation, and 13 degree external rotation in 90 degree abduction and two segment of internal rotation lower than normal. Only one (2.8 %) patient had postoperative dislocation. Conclusion: Modifi ed Putt i-Platt procedure had an excellent outcome score with acceptable recurrence rate and loss of range of motion.
