Pars Plana vitrectomy in complicated Retinal Detachment with Proliferative vitreoretinopathy.

We performed three-port pars plana vitectomy in 68 eyes of 66 patients for retinal detachment complicated by proliferative vitreoretinopathy (PVR). Fifteen (22.72%) patients were only-eyed. In 38 (55.88%) eyes the surgery was performed as a primary procedure, while 30 (44.11%) eyes had vitrectomy as a secondary procedure. Forty-nine (74.24%) patients were male and 17 (25.75%) were female. Thirty-six (54.54%) patients were below forty years of age. Thirty-nine eyes were phakic, 24 aphakic and 5 eyes were pesudophakic. Nineteen patients did not know about the duration of retinal detachment. In 50 (73.52%) eyes retina was totally detached on presentation. Forty-eight (70.58%) eyes had grade C and D PVR. Retinal break could not be identified in 30 (44.11%) eyes prior to surgery. Silicone oil was used as internal tamponade in 45 eyes. Perfluorocarbon liquid (PFCL) was used during the surgery in 12 eyes. Lensectomy was performed as part of the procedure in 15 eyes. In 3 eyes corneal epithelium had to be denuded. Retinotomy and retinectomy were performed in 8 eyes. Silicone explants were used in all eyes operated on. Intraoperative retinal incarceration in scleral ports was the commonest complication. A single case of globe rupture was managed successfully. Followup ranged from one month to thirty-six months. Overall anatomical success was achieved in 46 eyes (67.64%). The visual status of 3160 and better was achieved in 37 eyes (54.41%).
