Correlation of Left Main Coronary Artery Disease With Carotid Artery Disease in patients under going CABG.

Background and purpose: Carotid Artery stenosis is a significant factor in increased risk of adverse cerebrovascular events in both medical and surgical patients. Patients with the Left Main Artery (LMS) stenos undergoing CABG the concurrent incidence of Carotid Artery disease is associated with higher risk of cerebral events . This study was under taken to assess the correlation between the incidence of LMS stenos and Carotid Artery stenosis in patients undergoing coronary artery bypass grafting (CABG). Results: In this study a sample of 99 patients undergoing CABG was taken. Of the 99 patients studied 84 were males and 15 were females. Ages varied between 45 to 70 years. 85 patients had significant LMS disease (stenosis>50%). 30 patients had significant carotid artery disease diagnosed on Duplex scan study. One patient had history of previous stroke and one patient had carotid bruit. 2 patients had carotid endarterectomy done. Of the LMS stenosis group 14 patients had 60% stenosis, 23 with 70%, 4 with 75%, 2 with 80%, 35 with 90%and one with 95% stenosis. Two patients died due to causes other than stroke. 5 patients required intra aortic balloon pump support and one patient went in to low cardiac output syndrome. Majority of patients had an ejection fraction around 50 % ± 10%. 57 patients had hypertension, 44 patients had diabetes mellitus, 26 were smokers, 4 were obese and 20 had positive family history. All patients got one or more grafts with most of them having LIMA to LAD n=79. Reversed Saphenous venous grafts were applied to the others. Conclusion: Our results conclude that there exists an association between carotid artery disease and left main coronary artery disease with a significant correlation to stenosis.
