Role of Ultrasonagraphy in the diagnosis of Hepatic Abscess in children and usefulness of Percutaneous Needle Aspiration under Ultrasound guidance in its treatment.

Ultrasonography is a useful primary imaging modality in the early and rapid diagnosis of hepatic abscess and percutaneous needle aspiration under its guidance is safe and effective procedure A prospective study was carried out at the Department of Paediatrics, King Edward Medical College & Institute of Child Health, Lahore from March 1, 1999 to November 30, 2001. The study included 40 children of hepatic abscess confirmed by ultrasonography and subsequently percutaneous aspiration was done in 30 hepatic abscesses of a size > 5 cm for one to three times as necessary. They were all pyogenic in nature and the commonest organism was Staphylococcus aureus (70%). Ten children (25%) had hepatic abscess < 5 cm in size. These were treated conservatively with antibiotics and metronidazole. Ultrasonography proved its efficacy for detection of hepatic abscess and effectiveness in its treatment. Percutaneous needle aspiration of pyogenic abscess under ultrasound guidance is a safe treatment modality for treatment of hepatic abscesses >= 5 cm in size.
