Erectile dysfunction in Diabetics with increasing duration of disease.

There is substantial evidence that erectile dysfunction increases with duration of diabetes mellitus 1,7,14 We studied 910 male diabetics both type 1 and 2, more than 14 years of age. These were categorized according to their clinically known duration of disease. Mean age, fasting blood sugar level, two hour postprandial glucose level, HbA1C and frequency of patients with erectile dysfunctin in each group were comparable (table 1, figure 1,2,3). We found erectile dysfunction significantly related to glycemic control (2 hr postprandial glucose level p 0.007, HbA1C p 0.027), age of the patient (p<0.01) and other features of peripheral neuropathy (numb feet p 0.041, burning feet p 0.000, neuropathic pain p 0.009 and hammer toes p 0.021) only during first five years of clinically known duration of disease. During later years of disease erectile dysfunction appeared to be unrelated to above mentioned parameters. Though erectile dysfunction prevails in later groups in almost same frequency as during early years but appeared to be unrelated to parameters of diabetes mellitus or other features of neuropathy. These cross sectional correlates in diabetics may indicate that erectile dysfunction, an early feature of autonomic neuropathy may appears in early years of disease and in later years its presence appears to be unrelated to metabolic control of diabetes and its complications.
