Laparoscopic Varicocelectomy Versus Palomo Varicocelectomy in Men with Symptomatic Varicocele (Experience at North West General Hospital, Peshawar).

Objectives: To compare the success rates and morbidity of laparoscopic varicocelectomy (LV) compared to Palomo varicocelectomy (PV) by single surgeon (Aziz ur Rahman) at North West General Hospital Peshawar. Methods:Patients with idiopathic Varicocele diagnosed by clinical examination and Doppler ultrasonography with clear indications for surgery were admitted in our hospital and underwent surgery. Patients who underwent Laparoscopic varcocelectomy (LV) were put in Group A and those with Palomo Varicocelectomy (PV) were put in group B (with 21 patients in each group). All the procedures were conducted by Dr. Aziz Ur Rahman in same hospital. All patients were followed up at 3rd month and then at 6 months interval for 1 year. Operation time, Post OP pain, Hospital stay, complications, recurrence and persistence of the varicocele were recorded in all the patients of both groups. Results: Mean age was 27.2 � 3.4 in group A and 25.1 � 2.1 in group B. Mean operative time for group A was 20.1 � 9.5 while group B was 24.3 � 9.7 minutes, mean hospital stay was the same for both groups i.e. 1.5 days. Mean pain score at 0 and 1st post operative day in Group A were (1.2� 0.83, 0.6 � 0.74) and Group B were (7.2�1.14 and 3.5�0.96) which was significantly higher in group B as compared to Group A. The incidence of hydrocele was significantly higher in Group B compared to Group A (9.5% vs 0%) and the persistent or recurrence rates among the group A and B were 4.76% and 0% respectively. Conclusions: LV is better surgical modality as compared to IV in management of varicocele in terms of less hospital stay, post-operative pain and less complication rates. In expert hands LV is a better modality for management of varicocele in males.
