Gastric Pull-Up reconstruction for Laryngo-pharyngo-esophagectomy.

The objective of this study is to determine the results of gastric pull-up reconstruction following Laryngo-pharyngoesophagectomy. Retrospective review of 37 patients who underwent gastric pull-up reconstruction for carcinoma of hypopharynx and cervical esophagus during 6 years period from January 1996 to December 2001. Surgical procedures were performed by surgeons of Cardiothoracic and ENT & Head and Neck Surgery departments of Postgraduate Medical Institute, Lady Reading Hospital, Peshawar. Morbidity, mortality, time to oral feeding, length of hospitalization was evaluated. The mortality rate was 8.1%, perioperative morbidity rate 27% and medical morbidity 29.7%. Most of patients took oral feedings within 6 days after surgery and discharged with in 16 days. There were two temporary fistulas and four strictures, none of which was permanent. There was no intrathoracic or mediastinal injures or hemorrhage. The overall survival for one year was 100%. The gastric pull-up reconstruction is a safe, effective operation with low mortality rate and excellent long term functional results for patients with extensive carcinoma of hypopharynx and cervical esophagus.
