To determine the incidence of cancer in palpable abnormal prostate cancer, this study was conducted in the Department of Urology and Renal Transplant, Jinnah Hospital, Lahore from January 1999 to Jan 2003. This is a prospective study. Total of 117 patients with abnormal prostate on digital rectal examination were subjected to transrectal ultrasound., The suspected tissue corresponding to the area of palpable abnormalities was biopsied and histological examination was done. Of the 117 patients with abnormal digital rectal examination, the cancer detection rate was 63%. Palpable abnormalities more often were hypo-echoic. The positive biopsy rate for hypoechoic area was 77%, hyper-echoic 44% and for iso echoic area was 37%. Complications of biopsy were minimal. Palpable abnormal prostate is not a cancer in every case. Trans rectal ultrasound with combination of palpably abnormal prostate represents a method to diagnose cancer irrespective of the echogenicity of the abnormal area.
