How necessary is the use of Synthetic Mesh Repair in Ventral Abdominal Hernia?.

This prospective study included 105 patients with ventral abdominal herniae who underwent operation with different techniques. The purpose of the study was the application of modern mode of tension free mesh repair in ventral abdominal hernia and to determine the frequency of the usage of synthetic mesh for repair of ventral abdominal herniae. The inclusion criteria were patients above 12 years of age both males and females with clinical evidence of herniae of the ventral abdominal wall. Diabetes, hypertensives, hepatic cirrhotics and those afflicted with chronic illness were excluded from this study. Patients below 12 years of age were also excluded. Diagnosis was based on history and clinical examination. On the early post operative complication rate was 50%. In mesh repair it was 70% as compared to the in the simple repair, which was 30%. The incidence was high in obese patients. In this study one recurrence in mesh repair 1(16.6%) and in other repairs (Simple, Mayo & Keel repair) the recurrence to place in 11(9%) cases. Postoperative complications are more in mesh repair than in simple repair. The Mesh repair was used in larger hernie, required longer operating time was associated with greater blood loss than the simpler repair. Otherwise, the hernia should be repaired simply.
