Fetal and Maternal outcome of Preterm Premature Rupture of Membranes with Antibiotics.

Background: To determine the effect of prophylactic antibiotics in preterm premature rupture of membranes (PPROM) on maternal and fetal outcome. Methods: This prospective study was conducted at Holy Family Hospital in Gynae/Obs Unit Il. It comprised a total of 60 patients, either booked or non-booked, of any age and parity, between 28-34 weeks of gestation having PPROM. After confirmation of PPROM, injectable antibiotics were given for 48 hours, followed by oral antibiotics for 5 days. Signs of chorioamnionitis, mode of delivery, and fetal and maternal outcome were recorded. Duration from rupture of membranes to delivery, Apgar score at one and five minutes and follow up of neonates for neonatal sepsis or respiratory distress syndrome were recorded. Results: The maximum interval from rupture of membranes to delivery in this study was 17 days. 63.3% women had latent period < 3 days. The rate of development of chorioamnionitis was 8.3% and neonatal sepsis 2.9% while 3.33% of mothers developed puerperal pyrexia. Conclusion: There is improvement in overall fetal and maternal outcome, when prophylactic antibiotics are recommended for expectantly managed patients of PPROM between 28-34 weeks of gestation.
