Pak J Radiol Jan ;15(3):49-53.

A 55 years old male presented in the surgical emergency of Jinnah hospital Lahore with pain in the right lumber region/ Right iliac fossa for last two months off & on. He also complains of fever off and on for last 2-3 months. He got intermittent treatment from one of the rural health centre in Manga Mendi Lahore. He was routinely investigated in emergency department by routine laboratory investigations, plain X-Ray abdomen and ultrasound abdomen. The laboratory investigations were within normal limits. The plain x-ray abdomen shows two small radiopaque stones in the line of right ureter. He was send to Radiology Department for ultrasound to see the status of kidneys. Other than the ureteric stones with retrospective obstructive uropathy, bilateral renal cortical cysts were also noted. There was another incidental finding on ultrasound which revealed a long 11 X 5 cm well defined tubular mass with blind distal end were noted in the right iliac fossa. The mass was mobile while pressing with the ultrasound probe. This tubular structure shows typical Onion skin appearance. There was no evidence of any free fluid in the vicinity & in pelvis. No evidence of enlarged mesenteric lymph nodes noted in Right Iliac fossa. This tubular mass was clearly separated from the iliac vessels & ileal loops. This structure was diagnosed as Mucocele of Appendix. Surgery was done in the surgical unit - 4 of Jinnah hospital Lahore. On surgery there was large 11-12 cm tubular mass seen in the right iliac fossa which is freely mobile. The mass was surgically removed. On dissection there was whitish thick secretion seen coming out of the tubular mass. Histopathology confirmed Mucocele of Appendix.
