Acute Traumatic Extradural Hematoma - A Study of 80 Cases.

This is a retrospective study of 80 patients admitted with extradural haematoma at department of Neurosurgery, BV Hospital, over a three years period. Objective was to evaluate causes of morbidity and mortality and factors influencing the outcome. Patients were categorized in five groups on the basis of conscious level which was assessed according to glasgow coma scale. CT scan was done in all the cases. Cause of extradural haematoma was falls in 30-patients, assault in 28 and traffic accident in 24. Patients were treated by craniectomy or craniotomy and were followed up for 3months. Death rate was 11.25% and 3.75% patients became persistently vegetative. All these patients had glasgow coma score of 4 and faced dilated pupils at the time of surgery. Mortality was also higher in patients who had intradural pathology. Conclusion: Level of conscious at the time of surgery is the single most important decisive factor in the outcome, hence early diagnosis and surgical intervention is essential.
