12 year`s experience of Aortic Valve Replacement for Aortic Regurgitation.

Aortic Valve Replacement (AVR) is a standard procedure carried out due to severe pathological changes in aortic valve. In our study 42 patients had AVR for aortic regurgitation from Dec 1989 to Dec 2002. There were 38 males and 4 females. Most of the patients were from poor socioeconomic group. Rheumatic heart disease was the most common cause (95.2%). NYHA class III or IV with dilated left ventricle was the major indication of surgery. The mean age was 27 years. Starr Edward Model 12160 was most commonly used valve (61.9%) LV diastolic diameter was associated with increased pre-operative NYHA grading, increased inotropic support during the post operative period and hospital mortality. Hospital mortality was 4.8% and late deaths were 7.1%. 28.6% of patients were lost the follow up while 59.4 % are attending the follow up clinic with most of the cases in NYHA grade 0/I. Redo valve surgery and postoperative arrhythmias have affected the mortality significantly.
